In many cases, occlusal fissures may appear suspicious due to subsurface staining or decalcification, even when an explorer does not “stick” and radiographs show no visible evidence of decay. Because enamel thickness can mask underlying lesions, traditional diagnostic tools may fail to detect early caries. A cross-sectional view of such a tooth often confirms these suspicions, revealing lesions deep within the enamel that are invisible on radiographs.
